Most people in the United States are prone to a heart attack. There are 1 billion people in this world that do not develop this disease. However, most are not in the United States or eating our Western diet. It is very rare that I encounter a person that I deem has low enough risk not to be treated and some of these people were overweight.
The answer includes the person's blood pressure, family history, advanced ultrasound imaging but most of all the advanced lipoprotein analysis and genetic blood tests.

Some people's coronary artery lining can be exposed to much greater insult and not develop a vulnerable plaque and others with minor abnormalities can develop a vicious vulnerable plaque that can be lethal. Each person's threshold is different.
